Think you’re too old to have your teeth straightened? In reality, healthy teeth can be moved at any age to better positions. For the many adults who have some form of malocclusion (bad bite), orthodontics is still a viable option even in later years.

As important as it is to self-image and confidence, treating misaligned teeth can benefit you more than just improving your smile. Misaligned teeth may be harder to keep clean, setting up a mouth environment advantageous to the development of tooth decay or periodontal (gum) disease, the risks of both rising with age. And normally aligned teeth are easier to chew with than those misaligned.

Age isn’t the determining factor for whether you’re a good candidate for orthodontics — but your dental health is. Gum disease in particular can cause supporting bone loss, which can complicate orthodontic treatment. It’s important then that we first perform a complete oral examination and attempt to treat problems such as decay or gum disease first before attempting tooth movement.

What type of orthodontic treatment you’ll need will depend on the type of malocclusion you have and its relationship to the way your jaws fit together. Because your adult jaws have fully developed you may need orthognathic (“jaw straightening”) surgery to address certain advanced forms of malocclusion. If your bite problems aren’t that severe (the majority of situations) they can be treated with braces or, an increasingly popular alternative, clear aligners. These customized clear plastic trays are nearly invisible compared to metal braces and are effective for most patients.

Following the completion of tooth movement and other bite procedures, you will most likely need to wear a retainer to help prevent the teeth from reverting to their older positions. You may need to wear the retainer for a longer period than a younger patient, or perhaps indefinitely. Even with this mild inconvenience, though, you’ll still experience the positive effects of healthier and better functioning teeth and a great new smile.

There’s really no secret to keeping your child’s teeth healthy — good, daily hygiene habits, regular dental visits and early treatment for emerging problems. It’s a lot easier for those things to happen if your child feels comfortable with dental care and visiting the dentist. Sadly, that’s not always the case: many children develop an unhealthy fear of the dentist because the initial relationship may have been mishandled.

Here, then, are 3 tips that will help you foster a healthy relationship between your child and their dentist.

Visit the dentist before their first birthday. From a health standpoint, dental visits should begin soon after your child’s first teeth emerge (erupt) in the mouth. Visiting the dentist by their first birthday also improves the chances they’ll develop a sufficient level of comfort with the visits, more so than if you waited a year or two longer.

Choose your dentist with your child’s sense of security and comfort in mind. When you’re looking for a dentist to care for your child, think of it as looking for a “new member of the family.” It’s important to find an office environment that’s kid-friendly and staff members that work well with children. Some dentists specialize in pediatric dentistry and many general dentists have additional training in working with children. The key is a dental team that has a good, trust-building rapport with children.

Set an example, both in the home and at the dentist. Children learn quite a bit watching what their caregivers say and how they react in potentially stressful situations. If dental care is important to you personally, it’s more likely to become important to your child. And when you visit the dentist with your child, be sure to project calm and a sense that it’s routine — if you display tenseness or nervousness your child may take that as a sign that visiting the dentist is something to fear.

You want your child to learn that the dentist is their friend who’s there to help them. That lesson should begin early with the right dental team — and by making dental care a priority in your own life.

Sure, it’s big news when celebs tweet selfies from the dental office… if you’re still living in the 20th century. But in Hollywood today, it’s harder to say who hasn’t posted snaps of themselves in the dentist’s chair than who has. Yet the pictures recently uploaded to Twitter by Mark Salling, the actor and singer who regularly appears as Noah “Puck” Puckerman on the popular TV series Glee, made us sit up and take notice.

“Getting my chipped tooth fixed. Also, apparently, I’m a big grinder,” read the caption. The photo showed a set of upper front teeth with visible chips on the biting surface. What’s so special about this seemingly mundane tweet? It’s a great way of bringing attention to a relatively common, but often overlooked problem: teeth clenching and grinding, also called bruxism.

Although bruxism is a habit that affects scores of people, many don’t even realize they have it. That’s because the condition may only become active at night. When the teeth are unconsciously ground together, the forces they produce can wear down the enamel, cause chipping or damage to teeth or dental work (such as veneers or fillings), or even loosen a tooth! While it’s common in children under 11 years old, in adults it can be a cause for concern.

Sometimes, mouth pain, soreness and visible damage alert individuals to their grinding habits; other times, a dental professional will notice the evidence of bruxism during an exam or cleaning: tooth sensitivity and telltale wear and tear on the chewing surfaces. Either way, it’s time to act.

Bruxism is most often caused by stress, which can negatively impact the body in many ways. It may also result from bite problems, the overuse of stimulating substances (caffeine, alcohol, tobacco, and illegal drugs), and as a side effect of certain medications. Sometimes, simply becoming aware of the habit can help a person get it under control. Common methods of stress reduction include exercise, meditation, a warm bath or a quiet period before bedtime; these can be tried while we monitor the situation to see if the problem is going away.

If stress reduction alone doesn’t do the trick, several other methods can be effective. When bruxism is caused by a minor bite problem, we can sometimes do a minor “bite adjustment” in the office. This involves removing a tiny bit of enamel from an individual tooth that is out of position, bringing it in line with the others. If it’s a more serious malocclusion, orthodontic appliances or other procedures may be recommended.

When grinding is severe enough to damage teeth or dental work, we may also recommend a custom-made night guard (occlusal guard), which you put in your mouth at bedtime. Comfortable and secure, this appliance prevents your teeth from being damaged by contacting each other, and protects your jaw joints from stresses due to excessive grinding forces.

People with poor hygiene habits can develop a chronic form of periodontal (gum) disease known as gingivitis. Characterized by inflamed and bleeding gums, gingivitis is caused by an infection triggered by bacterial plaque, a thin film of food remnant built up on tooth surfaces.

This chronic form of gingivitis, though, can quickly escalate into more serious forms of gum disease that may lead to tooth and bone loss. One such condition is Acute Necrotizing Ulcerative Gingivitis (ANUG), also known as “trench mouth.” ANUG is a painful condition that can appear suddenly and result in extensive tissue damage and ulcerations, particularly in the papillae, the small, triangular bits of tissue between teeth. Persons with ANUG may also develop a foul breath and taste.

Gingivitis often develops into ANUG when certain mouth conditions exist: poor diet, smoking, which can dry the mouth and disrupt healthy bacterial flora, and increased stress or anxiety. If caught early, though, ANUG is highly treatable and reversible.

After determining you have ANUG and not another condition, our first step is to relieve the symptoms with non-steroidal anti-inflammatory drugs (NSAIDs) like ibuprofen to manage pain and reduce swelling. We also prescribe a regimen of antibiotics like Amoxicillin (a proven antibiotic against the specific bacteria that cause ANUG). This should decrease the symptoms within 24 to 48 hours.

As the inflammation subsides we want to continue treatment by removing any plaque or calculus (hardened plaque deposits), especially in hard to reach places. This involves a technique known as scaling in which we used specialized hand tools or ultrasonic equipment to manually remove and flush away plaque and calculus.

The final step depends on you. To prevent reoccurrence, it’s important for you to consistently practice effective oral hygiene to remove plaque — brushing twice and flossing once each day, and visiting us at least twice a year for cleanings and checkups. Quitting tobacco and improving your diet will also reduce your risk for ANUG.

ANUG and any other form of gum disease can cause a lot of damage. But taking steps to care for your teeth will help keep this acute form of gingivitis from arising in the first place.
